Parent's Information

Acknowledgment of Participation: Parents / guardians and players should be aware of the possible risks that are inherent in the nature of some of the activities. These risks include, but are not limited to, the potential for accidents or illness while participating. Every attempt is made to minimize the existing risks through proper sports equipment, safe facilities and sound safety practices. However, parents / guardians and players should realize these risks cannot be eliminated. By submitting registration form, the player and the parents / guardians understand that the player is participating in Valentia FC training at the player’s own risk and hereby agree, indemnify and hold harmless the Valentia Football Club from any liability in connection with the activity. In addition, the player and the parents / guardians understand that the Valentia Football Club carry no medical or accident insurance that will benefit the player, should the player be injured during the course of the program.

Quit Claim and Waiver: I, the parent / guardian of the registering player, affirm that I am aware of my son’s / daughter's physical condition and that he / she is voluntarily participating in the Valentia FC training. I am aware that such participation may result in possible injury as a result of his / her involvement in the sport. In addition, I hereby release all the playing venue managements of any and all responsibility in case of any personal injury sustained by my son / daughter or damage to property of others caused by my son / daughter while participating in the football activities. I have read and understand the above statements and will abide by them.*
